Understanding Flu Treatment with Tamiflu

What is Tamiflu?

Tamiflu (oseltamivir) is an antiviral medication that can help treat influenza (flu) when started early. It works against both Influenza A and B, but does not treat COVID-19, RSV, or the common cold.

When Your Provider May Prescribe Tamiflu

If Your Symptoms Started Recently (Within 48 Hours)

Tamiflu works best when started within the first 2 days of feeling sick. It can help you feel better faster and reduce the risk of complications.

The sooner you start, the better it works

If You Have Certain Health Conditions

Your provider may recommend Tamiflu even if it's been more than 48 hours since your symptoms started if you have any of these conditions:

  • • Age 65 or older, or under 2 years
  • • Pregnancy or recently gave birth
  • • Asthma, COPD, or lung conditions
  • • Heart disease or diabetes
  • • Kidney, liver, or neurologic disease
  • • Weakened immune system
  • • BMI of 40 or higher
  • • Living in a nursing home or long-term care

If Your Symptoms Are Severe or Getting Worse

Tamiflu is recommended for severe flu symptoms, regardless of when they started. This includes:

  • • Difficulty breathing or shortness of breath
  • • Symptoms that rapidly get worse
  • • Signs of pneumonia or other complications

During Flu Season with Flu-Like Symptoms

If the flu is spreading in your community and you have symptoms, your provider may start treatment before test results are available—especially if you're at higher risk or very sick.

When Tamiflu May Not Be Needed

If you're otherwise healthy with mild flu symptoms that started more than 2 days ago, your body can usually fight off the flu without antiviral medication. Your provider will discuss what's best for your situation.

What is Paxlovid?

Paxlovid (nirmatrelvir/ritonavir) is an antiviral medication that helps treat COVID-19 when started early. It's designed to keep mild to moderate COVID-19 from becoming severe and requiring hospitalization.

When Your Provider May Prescribe Paxlovid

All three conditions below must be met for Paxlovid to be prescribed:

Your Symptoms Started Within the Last 5 Days

Paxlovid must be started within 5 days of when you first felt sick. The sooner you start, the better it works at preventing your illness from getting worse.

You Have Mild to Moderate COVID-19 Symptoms

Paxlovid is for people who are sick but not hospitalized. If you're having symptoms like fever, cough, fatigue, or loss of taste/smell — but you're able to stay home — this medication may help prevent your symptoms from becoming severe.

Goal: Stop COVID-19 from getting worse and keep you out of the hospital

You Have at Least One High-Risk Factor

Paxlovid is recommended if you have any condition that puts you at higher risk for severe COVID-19. This includes:

Common High-Risk Factors:
  • Age 50 or older (especially 65+)
  • Not fully vaccinated or boosted
  • Diabetes
  • Heart disease or high blood pressure
  • Lung conditions (asthma, COPD)
  • Obesity (BMI 30 or higher)
  • Kidney or liver disease
  • Weakened immune system
  • Pregnancy
  • Cancer treatment
  • Neurologic conditions
  • Sickle cell disease

Even if you're vaccinated, Paxlovid can still be helpful if you have other risk factors

What If I Don't Have a COVID Test?

If COVID is spreading in your community and you have typical symptoms, your provider may recommend Paxlovid even before test results are available—especially if you're at higher risk. Don't delay scheduling your appointment if you're having trouble getting tested.

When Paxlovid May Not Be Safe

Your provider will review your medical history and current medications to make sure Paxlovid is safe for you. It may not be an option if you have:

  • Severe kidney problems (your provider will check your kidney function)
  • Severe liver disease
  • Medications that interact with Paxlovid and can't be safely paused or adjusted

What to Know If You're Prescribed Paxlovid

  • • Take twice daily — morning and evening
  • • Can be taken with or without food
  • • Significantly reduces risk of hospitalization
Possible Side Effects
  • Altered taste (metallic or bitter taste) — common and temporary

  • • High blood pressure (rare)
